We’ve all heard the buzzwords around being patient-centric and population-centric and now is the time to focus on building those relationships with patients to ensure they stay engaged in their healthcare journeys.

If we want to successfully make the transition from fee to value-based care, we need to make patients the center of everything we do. It is imperative that we gain a complete understanding of our patient-populations’ health needs, access to care, healthcare preferences and more so we can create conditions for them to succeed.

We need to move beyond the EHR and clinical data to capture the full picture of patient-populations. Nontraditional data sources should be considered as offering a new perspective to how patients act, feel and think regarding their own healthcare choices. The voice of the consumer will be paramount in understanding how they make decisions. Data that comes from wearables, analytics and other data sources will be increasingly valuable as the healthcare world becomes patient-centric
